1. Log in www.myuhc.com, and click Extra Programs and Discounts at lower right of the screen.
2. Choose UnitedHealth Allies, and click Wellness from the menu items located at the top of the page.
3. Click Fitness Clubs & Activities in the middle of the screen.
4. The search location may be changed by entering a zip code or city/town and state in the Find a Provider box located toward the top of the page and then clicking the Search button.

Gold's Gym:

• Discounted enrollment fee of $49 (normally $79-$199)
• 20% off monthly dues.

Anytime Fitness:
