Check the status of your Physical Activity Tracker

Enter your UnitedHealthcare Subscriber/Member ID (found on your UHC card) below to verify if your Physical Activity Tracker has been received.

If you are having difficulty verifying that your tracker was received you can call 222-6270. PLEASE NOTE: If your subscriber ID cannot be located you will need to provide proof of program completion (either fax or web page confirmation).
